TELMISARTAN HYDROCHLOROTHIAZIDE

Telmisartan hydrochlorothiazide is an anti-hypertension medicine that is a combination of two drugs– telmisartan, an angiotensin receptor, and hydrochlorothiazide, a thiazide diuretic. This drug is used to treat high blood pressure.

Telmisartan accomplishes its functions by blocking the vasoconstricting effects of angiotensin II by binding to receptors. Hydrochlorothiazide exerts its effects on the distal tubules of the kidney to increase the secretion of water, sodium and chloride ions. It decreases the reabsorption of electrolytes from the kidney tubules.

Telmisartan hydrochlorothiazide is used in the treatment of the following:

Do not give Telmisartan hydrochlorothiazide in people with the following conditions:

  • Hypersensitivity to telmisartan, hydrochlorthiazide, or sulphonamide-derived drugs
  • Cholestasis and biliary obstructive disorder
  • Anuria
  • Refractory hypokalemia
  • Hypercalcemia
  • Severe kidney and liver impairment
  • Pregnancy and lactation
  • Concomitant use with aliskiren-containing products in patients with diabetes or kidney disease

Telmisartan hydrochlorothiazide may have the following side effects:

  • Electrolyte disturbances
  • Renovascular hypertension
  • Hypotension
  • Increased cholesterol and triglyceride levels
  • Hyperuricemiaor gout
  • Acute transient myopia
  • Angle-closure glaucoma
  • Photosensitivity
  • Systemic lupus erythematosus activation or exacerbation
  • Kidneyfunction deterioration
  • Hypersensitivity reactions
  • Drowsiness
  • Dizziness
  • Fatigue
  • Flu-like symptoms
  • Insomnia
  • Headache
  • Migraine
  • Anxiety
  • Depression
  • Syncope
  • Paresthesia
  • Chest pain

Precautions

Take extra caution in patients with the following medical conditions:

  • Intravascular hypovolemia
  • Severe heart failure
  • Renal artery stenosis
  • Primary aldosteronism
  • Aortic and mitral stenosis
  • Obstructive hypertrophic cardiomyopathy
  • Diabetes mellitus
  • Hypercholesterolemia
  • Parathyroid disease
  • Systemic lupus erythematosus
  • History of bronchial asthma or gout
  • Mild to moderate hepatic and renal impairment

 

Interactions

Telmisartan hydrochlorothiazide may interact with the following medicines:

  • Lithium
  • Potassium-sparing diuretics
  • Potassium supplements, potassium-containing salt substitutes
  • ACE inhibitors
  • Cyclosporin
  • Pressor amines
  • NSAIDs
  • Other antihypertensive agents
  • Digoxin
  • Corticosteroids
  • ACTH
  • Amphotericin
  • Carbenoxolone
  • Salicylic acid derivatives
  • Kaliuretic diuretics
  • Anionic exchange resins (e.g. cholestyramine, colestipol)
  • Nondepolarizing skeletal muscle relaxant (e.g. tubocurarine)
  • Beta-blockers and diazoxide
  • Amantadine
  • Cytotoxic agents (e.g. cyclophosphamide, methotrexate)
  • Anticholinergic agents (e.g. atropine, biperiden)
  • Calcium-containing medicinal products
  • Aliskiren

Symptoms of overdose include the following:

  • Hypotension
  • Tachycardia
  • Dizziness
  • Bradycardia
  • Vomiting
  • Increased serum creatinine
  • Acute renal failure
  • Electrolyte depletion (e.g. hypokalaemia, hypochloraemia)
  • Hypovolemia
  • Nausea
  • Somnolence
