UMECLIDINUM BROMIDE VILANTEROL

Umeclidinium is a long-acting quaternary ammonium antimuscarinic substance. It intensely and reversibly hinders acetylcholine binding at type 3 muscarinic (M3) receptors in the bronchial smooth muscles, thus causing bronchodilation.

Vilanterol is a specific, long-acting β2-adrenergic agonist. It stimulates the release of cyclic adenosine-3′,5′- monophosphate (cAMP) by initiation of adenyl cyclase which results in the relaxation of bronchial smooth muscle.

Umeclidinum Bromide Vilanterol is used in the following:

Maintenance therapy in chronic obstructive pulmonary disease (COPD)

Do not give Umeclidinum Bromide Vilanterol to people with the following conditions:

  • Asthma
  • Acute episodes of bronchospasm
  • Acutely deteriorating COPD
  • Concomitant use with other long-acting beta-2-agonists

Umeclidinum Bromide Vilanterol may have the following side effects:

  • Hypersensitivity reactions (e.g. anaphylaxis, angioedema, rash, urticaria)
  • Increased blood pressure and tachycardia
  • Atrial fibrillation
  • Hyperglycaemia
  • Diabetes mellitus and ketoacidosis exacerbation
  • Hypokalaemia
  • Urinary retention
  • Narrow-angle glaucoma
  • Bladder outlet obstruction
  • Central nervous system stimulation/excitation
  • Thyrotoxicosis exacerbation
  • Gastrointestinal disorders
  • Constipation
  • Dry mouth
  • Diarrhea
  • Abdominal pain
  • Nausea
  • Toothache
  • Musculoskeletal and connective tissue disorders
  • Muscle pain
  • Pain in extremity
  • Neck pain
  • Nervous system disorders
  • Headache
  • Vertigo
  • Tremor
  • Dysgeusia
  • Urinary tract infection
  • Thoracic and mediastinal disorders
  • Sinusitis
  • Nasopharyngitis
  • Pharyngitis
  • Upper respiratory tract infection
  • Cough
  • Pleuritic chest pain

Precautions

Take extra caution in patients with the following medical conditions:

  • Patient with heart disease (e.g. cardiac insufficiency, arrhythmias, hypertension)
  • Diabetes mellitus
  • Narrow-angle glaucoma
  • Hypokalaemia
  • Prostatic hyperplasia/bladder neck obstruction
  • Seizure disorders
  • Thyrotoxicosis
  • Concomitant use with other long-acting muscarinic antagonists
  • Pregnancy and lactation

 

Interactions

Umeclidinum Bromide Vilanterol may interact with the following medicines:

  • Other anticholinergic drugs.
  • Strong CYP3A4 inhibitors (e.g. ketoconazole, clarithromycin)
  • Methylxanthine derivatives
  • Steroids
  • Non-potassium-sparing diuretics
  • Beta-adrenergic blockers.
  • Monoamine oxidase inhibitors(MAOIs)
  • Tricyclic anti-depressants
  • Long-acting beta 2-adrenergic agonists (e.g. formoterol, salmeterol)

If you think you may have used Umeclidinum Bromide Vilanterol more than the usual dose, seek medical help immediately.
