TEGAFUR + GIMERACIL + OTERACIL

Tegafur, a fluoropyrimidine antimetabolite agent, is a prodrug of 5-fluorouracil that produces 5-fluoro-deoxyuridine-monophosphate as an active metabolite (FdUMP). FdUMP and decreased folate create a ternary complex with thymidylate synthase, which inhibits DNA synthesis.

Gimeracil increases 5-FU exposure and anti-tumor action by inhibiting dihydropyrimidine dehydrogenase (DPD), the major enzyme that degrades 5-FU.

Oteracil inhibits orotate phosphoribosyltransferase (OPRT), an enzyme hypothesized to be involved in 5-FU toxicity in the GI tract, lowering 5-FU toxicity in the GI mucosa.

Interactions

Tegafur + Gimeracil + Oteracil may interact with the following medicines:

  • Anticoagulants derived from coumarin
  • Clozapine
  • Cimetidine
  • Methotrexate
  • Nitroimidazoles
  • With CYP2A6 inhibitors and allopurinol, effectiveness is reduced. Phenytoin causes an increase in plasma levels and toxicity.

Inform your physician about the drugs you take including over-the-counter medications, prescribed drugs, supplements and vitamins.
