Sapropterin is an engineered type of tetrahydrobiopterin (BH4), an endogenous cofactor for phenylalanine hydroxylase (PAH). This PAH enzyme hydroxylates phenylalanine through an oxidative response to create tyrosine. PAH activity is missing or inadequate in patients with phenylketonuria. Sapropterin enhances the action of leftover PAH, thus improving normal phenylalanine metabolism and diminishing serum phenylalanine levels.
