Description: Norfloxacin is a fluoroquinolone anti-infective agent which inhibits DNA synthesis thereby stopping bacterial growth

Pharmacokinetics:

Absorption: Rapidly but incompletely absorbed from the GI tract (approx 30-40%). Food may delay rate and/or extent of absorption. Time to peak plasma concentration: Approx 1-2 hr.

Distribution: Penetrates well into tissues of genito-urinary tract; crosses the placenta and distributed into cord blood and amniotic fluid; relatively high concentrations occur in bile. Plasma protein binding: Approx 14%.

Metabolism: Undergoes partial hepatic metabolism to several metabolites.

Excretion: Via urine by tubular secretion and glomerular filtration w/in 24 hr (approx 30% as unchanged drug); faeces (approx 39%). Plasma half-life: 3-4 hr.

    • Bacterial Infection Treatment.

 

  • Belongs to a class of drugs called quinolone antibiotics that inhibits the growth of bacteria.

 

How to use norfloxacin

  • Always refer to your doctors instructions before taking this medication. Duration and dosage will be determined by your medical condition and its response to the medication. Make sure to take this mediciation in the prescribed amount and time period even after symptoms disappear. Discontinuing the medication before completion of treatment may cause incomplete treatment of condition. If Symptoms persist, consult your doctor

 

  • Taken by mouth; Take with a full glass of water(8 ounces or 240 milliliters) at least one hour before or two hours after a meal or dairy products; usually twice a day or as directed by your physician;

 

  • Take this medication at least 2 hours before or 2 hours after taking any of the following:
    • Drugs that contain magnesium, aluminum, or calcium
      • Quinapril, certain forms of didanosine (chewable/dispersible buffered tablets or pediatric oral solution), vitamins/minerals, and antacids

Calcium-enriched juice, bismuth subsalicylate, sucralfate, iron, and zinc

    • May rarely cause tendon damage (e.g., tendinitis, tendon rupture) during or after treatment. Your risk for tendon problems is greater if you are over 60 years of age, if you are taking corticosteroids (such as prednisone), or if you have had a kidney, heart or lung transplant. Stop exercising, rest, and seek immediate medical attention if you develop joint/muscle/tendon pain or swelling

    Should not be used in patients with myasthenia gravis. It may cause the condition to become worse. Seek immediate medical attention if you develop muscle weakness or trouble breathing.

      • Minor side effects (if these symptoms persist or worsen, inform your doctor)
        • Nausea, diarrhea, dizziness, lightheadedness, or headache

       

      • Serious side effects (inform your doctor immediately)
        • sunburn (sun sensitivity), unusual bruising/bleeding, signs of a new infection (e.g., new/persistent fever, persistent sore throat), unusual change in the amount of urine, signs of liver problems (e.g., unusual tiredness, stomach/abdominal pain, persistent nausea/vomiting, yellowing eyes/skin, dark urine), severe/persistent headache, vision changes, shaking (tremors), seizures, severe dizziness, fainting, fast/irregular heartbeat, mental/mood changes (e.g., anxiety, confusion, hallucinations, depression, rare thoughts of suicide).
        • May rarely cause serious, possibly permanent, nerve problems (peripheral neuropathy). Stop taking norfloxacin and tell your doctor right away if you have any of the following symptoms:
          • pain/numbness/burning/tingling/weakness in your arms, hands, legs, or feet, changes in how you sense touch/pain/temperature/vibration/body position.
        • May rarely cause Clostridium difficile-associated diarrhea) due to a type of resistant bacteria.
          • persistent diarrhea, abdominal or stomach pain/cramping, blood/mucus in your stool

       

      • Severe allergic reactions are rare. Get immediate medical help if you notice the following:

      Rash, itching/swelling (especially of the face/tongue/throat), severe dizziness, trouble.

      • Precautions:
        • Inform your doctor
          • Allergy to the medication or any allergies or to other quinolone antibiotics such as ciprofloxacin, gemifloxacin, levofloxacin, moxifloxacin, or ofloxacin
          • Medical History of certain metabolic disorder (G6PD deficiency), diabetes, heart problems (e.g., recent heart attack), joint/tendon problems (e.g., tendonitis, bursitis), kidney disease, myasthenia gravis, nerve problems (such as peripheral neuropathy), seizure disorder, conditions that increase your risk of seizures (e.g., brain/head injury, brain tumors, cerebral atherosclerosis)

         

        • May cause QT prolongation, a condition that affects the heart rhythm
          • Risk increases if you have the following conditions”
            • Heart problems, family history of certain heart problems
            • Low levels of potassium or magnesium
              • Usage of such as diuretics/”water pills” or presence of conditions such as severe sweating, diarrhea, or vomiting.

         

        • May rarely cause serious changes in blood sugar levels, especially if you have diabetes.

         

        • May cause dizziness or drowsiness. Limit alcohol intake

         

        • May cause light sensitivity (photosensitivity)
          • Avoid prolonged sun exposure, tanning booths or sunlamps
          • Sunscreen and protective clothing are recommended when outdoors.

         

        • Children younger 18 and older adults may be more sensitive to the side effects of this drug. Discuss the risks and benefits of use with your physician.

         

         

        • Should only be used when there is a clear need during pregnancy as it may harm an unborn baby. If you are planning to be pregnant or think you may be pregnant, discuss the risks and benefits of use with your doctor.

         

        • It is uknown if this drug passes into breast milk. Before breast-feeding, consult your physician.

         

        Interactions:

         

        Taking multiple drugs may affect how medications work and/or cause unwanted side effects. List down all the medications you are taking and inform your physician and pharmacist. Do not take, discontinue, or alter any dose of medications without prior approval of your physician.

         

        • Products that may interact with this medication include:
          • Do not use with Strontium
          • amiodarone, dofetilide, procainamide, quinidine, sotalol, certain macrolide antibiotics (e.g., erythromycin, clarithromycin), and certain antipsychotic medications (e.g., pimozide, thioridazine, ziprasidone), among others. Therefore, before using norfloxacin, report all medications you are currently using to your doctor or pharmacist.
          • live bacterial vaccines (e.g., typhoid, BCG), “blood thinners” (e.g., warfarin), corticosteroids (e.g., prednisone, hydrocortisone), drugs removed from your body by certain liver enzymes (such as clozapine, duloxetine, ropinirole, tacrine, tizanidine), nonsteroidal anti-inflammatory drugs (NSAIDs such as ibuprofen, naproxen), urinary alkalinizers (e.g., potassium/sodium citrate)
          • isoniazid (INH), phenothiazines (e.g., chlorpromazine), theophylline, or tricyclic antidepressants (e.g., amitriptyline),

        Avoid drinking large amounts of beverages containing caffeine (coffee, tea, colas), eating large amounts of chocolate, or taking over-the-counter products that contain caffeine

      • If overdose is suspected, contact a poison control center or emergency room immediately..Missed Dose

        If you miss a dose, take it as soon as you remember. If it is near the time of the next dose, skip the missed dose and As soon as you remember, take a regular dose or if the time of the next dose is near, wait for it before taking another dose and resume your usual schedule. Do not increase the dose to compensate for the missed dose.
