Description: Isosorbide mononitrate decreases left ventricular pressure (preload) and arterial resistance (afterload), and relaxes vascular smooth muscles by stimulating cyclic-GMP.

Isosorbide mononitrate prevents chest pain (angina) in patients with a certain heart condition (coronary artery disease) by relaxing and widening blood vessels. This medication will not relieve chest pain once it occurs. This medication may also be used with other drugs to improve symptoms of heart failure (such as trouble breathing).

How to use isosorbide mononitrate

Take this medication by mouth as directed by your doctor, usually twice a day. Each day, take the first dose when you wake up, then take the second dose 7 hours later. It is important to take the drug at the same times each day. Do not change the dosing times unless directed by your doctor.

Use this medication regularly to get the most benefit from it. To help you remember, take it at the same times each day. Do not stop taking it without consulting your doctor. Some conditions may become worse when the drug is suddenly stopped. Your dose may need to be gradually decreased.

Although unlikely, when this medication is used for a long time, it may not work as well and may require different dosing. Tell your doctor if this medication stops working well (for example, you have more frequent or worsening chest pain).

Tell your doctor or pharmacist promptly if any of these effects persist or worsen:

  • Headache
  • Dizziness
  • Lightheadedness
  • Nausea
  • flushing

Tell your doctor right away if you have fainting or fast/irregular/pounding heartbeat.

A very serious allergic reaction to this drug is rare.

  • Older adults may be more sensitive to dizziness and lightheadedness
  • This medication should be used only when clearly needed during pregnancy
  • Consult your doctor before breast-feeding

Interactions 

Drugs to treat erectile dysfunction-ED or pulmonary hypertension (such as sildenafil, tadalafil) and riociguat may interact with this drug.

Do not use medications containing isosorbide dinitrate while using isosorbide mononitrate.

This medication may interfere with certain laboratory tests (including cholesterol levels), possibly causing false test results.

Overdose

If overdose is suspected, contact a poison control center or emergency room immediately. US residents can call their local poison control center at 1-800-222-1222. Canada residents can call a provincial poison control center. Symptoms of overdose may include: slow heartbeat, vision changes, severe nausea/vomiting, sweating, cold/clammy skin, bluish fingers/toes/lips.

Missed Dose

If you miss a dose, take it as soon as you remember. If it is within 2 hours of the next dose, skip the missed dose and resume your usual dosing schedule. Do not double the dose to catch up.

Storage

Store at room temperature away from light and moisture. Do not store in the bathroom. Keep all medications away from children and pets.

Do not flush medications down the toilet or pour them into a drain unless instructed to do so. Properly discard this product when it is expired or no longer needed. Consult your pharmacist or local waste disposal company.
