DOFETILIDE

GENERIC NAME(S): DOFETILIDE

 

Description: Dofetilide, a class III antiarrhythmic agent.  It acts by selectively inhibiting repolarization of the cardiac cells.  It has negligible effects on heart rate or BP and slightly improve cardiac contractility.

Dofetilide is used to treat atrial fibrillation/flutter by restoring the normal heart rhythm and maintain a regular, steady heartbeat. It blocks the activity of certain electrical signals in the heart that can cause an irregular heartbeat. It can decrease the risk of blood clot and stroke.

How to use dofetilide

Take by mouth, usually twice daily with or without food or as directed by your doctor.

Althought this is an anti-arrhythmic drug, it may cause arrhythmia at some cases. Talk to your doctor about the risks and benefits of taking this drug.

Side effects may range from mild to serious complications.  Consult your doctor immediately when you feel the following symptoms: headache, dizziness, or nausea, chest pain, fainting, faster/more irregular heartbeat, severe dizziness.

Seek emergency medical help if you expereince allergies (rash, itching/swelling (especially of the face/tongue/throat), severe dizziness, trouble breathing) or the symptoms mentioned above while taking this drug.

Precautions

Inform your doctor of your allergies (food or drugs), medical conditions and  medical history before taking this drug, especially of: kidney problems, liver disease, certain heart problems (heart failure, slow heartbeat, QT prolongation in the EKG), family history of certain heart problems (QT prolongation in the EKG, sudden cardiac death).

Do not perform activities that require alertness such as driving and using machinery because thid drug can make you dizzy.

This drug may affect the heart rhythm referred to as QT prolongation. QT prolongation can infrequently result in serious (rarely fatal) fast/irregular heartbeat and other symptoms (such as severe dizziness, fainting). Consult your doctor right away.  Other conditions that may increase your risk are: low levels of potassium or magnesium, severe sweating, diarrhea, or vomiting. Talk to your doctor about using dofetilide safely.

Older adults may be more sensitive to the side effects of this drug, especially QT prolongation (see above).

For pregnant and nursing (breastfeeding) women, consult your doctor before using this medication.

 

Interactions 

Before using this drug, tell your doctor of the current medications, products (herbal/nonprescription) that you are taking especially of: dolutegravir, fingolimod, amiodarone, pimozide, procainamide, quinidine, saquinavir, sotalol, macrolide antibiotics (such as clarithromycin, erythromycin), and certain quinolone antibiotics (such as sparfloxacin), among others, hydrochlorothiazide, itraconazole, ketoconazole, megestrol, prochlorperazine, trimethoprim, verapamil, among others.

Avoid taking cimetidine while being treated with dofetilide. Ask your doctor or pharmacist about other medications for heartburn/upset stomach.

Overdose

Get medical emergency help immediately when you experience symptoms of overdose- dizziness, fainting.

Missed Dose

If you miss a dose, skip the missed dose and resume your usual dosing schedule. Do not double the dose to catch up.

Storage

Store at room temperature away from light and moisture. Do not store in the bathroom. Keep all medications away from children and pets. Discard the drug properly- do not throw in toilet or sink.
