DELAMANID

Delamanid is a nitroimidazole-oxazole drug that has activity against mycobacterial organisms. It acts through bioreduction of its nitro group by Mycobacterium tuberculosis. It restrains the creation of mycobacterial cell wall parts, methoxymycolic and ketomycolic acids.

Delamanid is a new medication that is presently classified as a D2 drug., and its use is recommended in adults for a maximum of six months. It can be added to an OBR (optimized background regimen) at the recommended doses, under adequate clinical monitoring that includes the measurement of QT intervals in ECG. The WHO has recently approved the use of this drug or children above six years of age, given that PK (pharmacokinetic) data having become successfully available.

 

This also helps to increase sputum smear and culture conversions to have success rates at the end of treatment, in adults, though much still needs more data to be gathered, especially with regards to drug-to-drug interactions and potential cardiotoxicity. Together with bedaquiline, as well as other core second-line drugs such as fluoroquinolones and clofazimine, it may increase the QT interval in ECG tracings.

 

 

Delamanid is used in the following conditions:

  • Pulmonary multidrug-resistant tuberculosis

Do not use Delamanid in people with the following conditions:

  • Hypersensitivity
  • Serum albumin less than 8 g/dL
  • Concomitant use with strong CYP3A4 inducers

Delamanid may have the following side effects:

  • Reticulocytosis
  • Hemoptysis
  • Palpitations
  • Tinnitus
  • Hyperlipidemia
  • Photophobia
  • Nausea
  • Oropharyngeal pain
  • Malaise
  • Hypokalaemia
  • Decreased appetite
  • Restlessness
  • Insomnia
  • Dyspnea
  • Dermatitis
  • Hypertension

Take extra caution in patients with the following medical conditions:

  • Patient with risk factors for QT prolongation
  • Heart attack
  • Severe hypertension
  • Left ventricular hypertrophy
  • Moderate to severe hepatic or severe renal impairment
  • Pregnancy
  • Lactation

Interactions

Delamanid may interact with the following medicines:

  • Ritonavir
  • Clarithromycin
  • Moxifloxacin
  • Amiodarone
  • Astemizole
  • Cisapride
  • Droperidol
  • Domperidone
  • Methadone
  • Carbamazepine
